Failure
to carry out a risk assessment led to a workplace transport
accident in which a 53-year old driver at Glebe Mines Ltd's
Derbyshire premises was fatally injured.
The
accident in March 2003, happened when the employee was collecting
a consignment of chemicals for another company, and was trapped
between a loading shovel and a fork-lift truck.

HSE investigation established that Glebe Mines had no risk
assessment and had not properly planned the movement of vehicles
and personnel at its site. The company was fined a total of
£25,000 plus costs after it admitted failing to ensure
the safety of the driver contrary to s.3(1) of the Health
& Safety at Work etc Act 1974, and failing to carry out
a risk assessment, contrary to r.3(1) of the Management of
Health and Safety at Work Regulations 1999.
